ABSTRACT

Abstract Objective To investigate the effects of combination therapy with cholecalciferol and lansoprazole on residual β-cell function and glycemic control in children with new-onset type 1 diabetes. Methods Children aged 6-12 years with type 1 diabetes were allocated to receive cholecalciferol and lansoprazole (Group 1) or no treatment (Group 2). Children were maintained on their respective insulin regimens and kept records of blood sugar and insulin doses taken. Children were followed at three-month intervals for six months. Changes in mean fasting C-peptide and HbA1c levels, daily insulin doses, fasting blood glucose and mean blood glucose levels from baseline to end of the study were analyzed. Results Twenty-eight children (14 per group) met the eligibility criteria. Fasting C-peptide levels decreased significantly from baseline to study end in both groups (mean decrease -0.19±0.09ng/mL and -0.28±0.08ng/mL, p=0.04 and p=0.001; Group 1 and Group 2 respectively). However, fasting C-peptide level drop was significantly smaller in Group 1 compared to Group 2 (30.6% and 47.5% respectively; p=0.001). Likewise, daily insulin doses decreased significantly in both groups (-0.59±0.14units/kg and -0.37±0.24units/kg respectively; p=0.001). All patients recruited completed the study. No adverse events were reported. Conclusion Combined therapy with cholecalciferol and lansoprazole for six months was associated with smaller decline in residual β-cell function and lower insulin requirements in children with new-onset type 1 diabetes. Preliminary findings of this small-scale study need to be confirmed by larger studies. Registry of Clinical Trials (www.ctri.nic.in) under number REF/2021/03/041415 N.

ABSTRACT

Background “Coronary slow flow’’ (CSF) is delayed vessel opacification in the absence of epicardial stenosis. Studies in different ethnic groups have found variable risk factors associated with CSF. Aim of present study was to analyze the risk factors and angiographic profile of CSF in North Indian population, not studied till date. Methods 40 patients with CSF and 40 controls were studied. CSF was determined quantitatively by thrombolysis in myocardial infarction (TIMI) frame count method. Various clinical risk factors (age, sex, body mass Index (BMI), diabetes, hypertension, dyslipidemia, smoking), hematological and biochemical parameters (hematocrit, platelet count, uric acid, homocysteine, fibrinogen, high sensitivity C reactive protein (hsCRP), glycosylated hemoglobin (HbA1c) were assessed. Results Of the 40 patients with CSF, 37 (92.5%) were males. While 20 patients (50%) presented with chronic stable angina, rest 20 (50%) presented with acute coronary syndrome. [15 (37.5%) with unstable angina and 5 (12.5%) with non ST elevation myocardial infarction (NSTEMI)]. Patients with CSF had significantly higher BMI (27.27 ± 2.82 vs. 24.12 ± 2.35, p < 0.001), fibrinogen levels (398.48 ± 120.96 vs. 331.55 ± 162.6, p = 0.04) and smoking (24(60.0%) vs 14(35.0%), p = 0.02). On multivariable regression analysis, only BMI was found to have an independent association with CSF (odds ratio 1.613, 95% confidence interval 1.265–2.057, p < 0.001). Conclusion This is the first study to analyze clinical presentation, angiographic profile and risk factors associated with CSF in North Indian population. In this study, we found only BMI to have an independent association with CSF.

ABSTRACT

The present study was aimed at screening the phytochemical contents, antimicrobial and antioxidative activities from the aerial parts of Solanum sisymbriifolium. S. sisymbriifolium is known to be traditionally used in various kinds of ailments including pain and inflammation, some of which may be due to infection by various microbes found in our environment. Phytochemical analyses revealed the presence of phenolics, flavonoids, tannins, alkaloids and saponins in this plant. Screening for the antimicrobial activity of tissue extracts by disc diffusion assay against test microorganisms revealed the presence of antimicrobial activity, maximum activity being found in the methanolic extracts. The activity was stable against drastic thermal and pH treatments. The antioxidative activity of its aerial tissue extracts was determined using DPPH-radical scavenging method. Antioxidant activities have been implicated as agents for fighting many other diseases such as cancer that result from oxidative damages arising out of synthesis of compounds via natural metabolic routes. The study suggests the use of Solanum sisymbriifolium species for further isolation and characterization of its bioactive compounds leading to the development of new antimicrobial drugs.

ABSTRACT

Antimicrobial activity of the aqueous and organic solvent extracts of the leaves of Pavetta indica were tested against Bacillus subtilis, Escherichia coli and Saccharomyces cerevisiae using disc diffusion assay. Most of the leaf extracts showed bactericidal activity against B. subtilis. None of the extracts exhibited any activity against E. coli and S. cerevisiae. Minimum inhibitory concentration (MIC), minimum bactericidal concentration (MBC), thermal stability and qualitative phytochemicals studies were performed. Both MIC and MBC of the aqueous and methanol extracts were found to be between 1.95 - 7.81 mg/ml. The activity of aqueous and methanol extracts were found to be stable despite thermal treatment. Phytochemical analysis of aqueous extract revealed the presence of flavonoids, saponins and carbohydrates. Methanol extract was found to be positive for saponin and cardiac glycosides. TLC and bioautography were also done to identify the active fractions responsible for the antimicrobial activities. Results showed the presence of a number of bactericidal components. The study suggests P. indica to be a source for isolation of antibacterial compounds for human health care and use as preservatives in food processing industry.
